It helps when YOU know the title you earned AND the spelling of your dogs name for the plaque!

To qualify for club awards, members must report each trial result (on an Achievement Form) and title earned within 4 weeks (28 days) of your accomplishment to qualify for any Top Dog standings. Past dated ladders are $4.00. Past dated plaques are $36.00.

AKC awards are complimentary to members, all other registries remit payment with this form (check payable to QCDTC).

If you are not available to pick up your award at a general meeting, please appoint a person to sign for the award: (this is only required if you are receiving a plaque or ladder)
